Medical Records Abbreviations

There are a lot of abbreviations that appear in medical records used in disability claims. Some examples some include:

HEENT - head, eyes, ears, nose and throat

WNL - within normal limits

VS - vital signs

HPI - history of present illness

CC - chief complaint

ROS - review of systems

PMH - past medical history

PSH - past surgical history

LOC - level of consciousness

NAD - no acute distress

DTR - deep tendon reflexes

R/O - rule out

MI - myocardial infarction

EMR - electronic medical record

PRN - patient return as needed
